Did the combine get to Xen yet?

The Vortigaunts are on Earth, one can only assume, because of the portal storms. Most of the ones you meet didn't have any shackles off because they had been slaves to the Nihilanth, and with it dead they were free.
However there are one or two who're still slaves to the Combine (at the very beginning, go to the right, and you'll see one with shackles on (albeit different ones to the ones they had in HL1) sweeping the floor.

The Combine weren't on Xen in HL1, but whether they are now or not, I don't know.

But this begs the question - if Kleiner and Eli have built teleporters that are capable of reaching Xen, why haven't the Vortigaunts stranded on earth started leaving?

The real Combine are over-evolved slugs. They are depedant on the technology that they created. But it doesn't matter as they have machines and slaves to do everything for them.

What you saw on Earth was just an ocupation force of modified Humans and a few Synths; what the Combine's army is made of. They are alien races that have been asimilated into the Combine's army.

I like to think of the Overwatch soldiers as the Human Synths, though still under development.

I vote no. There is so much teleportation technology on Xen (both Xenian and human) that the Combine would have been able to reverse-engineer it without needing help from Breen or Mossman.
e.g. the teleporters Freeman uses on Xen, the old BM teleporters Barney uses.

Besides, the Xenians were able to flee to Xen and were able to successfully establish themselves pretty well without Combine interference. That really suggests that the Combine couldn't get to them.

I know this. but it's safe to say their homeworld is screwed now. When the rest of the Vortigaunts are on Xen, why wouldn't the stranded ones want to join them?

But as Blue Shift showed, if you can teleport locally, you can teleport to Xen.

I'm not going to state my alleigance in this argument because it isn't easy to make one argument stand out over the other one, there is conflicting evidence for both. So...yeah. :)

Well, I doubt they were there during the HL1 period, as for the tenure of HL2 IMO it is impossible to state for sure the Combine haven't been on Xen. There is a small chance I could be enlightened, however.

Well...it isnt necesarily the HOMEWORLD of the combine overlord race. The combine controls many interdimensional and intergalactic planets right? That means it coulda been any planet the combine has control on.
